Enrollment withheld students who have been reinstated must complete a Progress Report to register for future terms. A successful Report includes:

1. Signatures of all instructors, including tentative grades.
2. Signature of program advisor and indication of support.
3. Tentative grades must meet the student's conditions of reinstatement.

Completed forms must be submitted to Lindsay in 230 Chrysler Center.
